最早的 "First Lady" 的稱(chēng)謂是由 "first lady of honor", "first lady of the Admiralty" 演化而來(lái)，用來(lái)這些稱(chēng)呼指那些有影響力的杰出女性。1834 年更出現(xiàn)了 "first lady in (of) the land"的稱(chēng)呼。 愛(ài)德華-布爾沃-里頓在1851年引用了這一說(shuō)法， "She was so beautiful and so good, and not proud she! Though she looked like the first lady in the land."
"First Lady"第一次被用來(lái)指總統(tǒng)夫人是1849年, 托卡里-泰勒總統(tǒng)稱(chēng)贊多利-麥迪遜夫人時(shí)說(shuō)道， "She will never be forgotten because she was truly our First Lady for a half-century." (希拉里－克林頓在她的1999年多利-麥迪遜夫人紀(jì)念銀幣發(fā)行儀式上的講話中引用了這句話。)
19世紀(jì)下半期，人們則很少用 "First Lady" 來(lái)指總統(tǒng)夫人，直到20世紀(jì)， "First Lady"才開(kāi)始流行。 1911年， 埃爾西-費(fèi)格森主演的名為 "Dolly Madison" (又名"The First Lady of the Land")的戲劇在紐約上演。1941年普林德維爾出版"First Lady"一書(shū)。 《牛津英語(yǔ)詞典》引用了《芝加哥論壇》的 "Mrs. Thomas E. Dewey discloses today what kind of first lady she will be."
